IZUMI Foundation cultivates partnerships with organizations to improve health and healthcare for children, families, and communities. With an emphasis on supporting organizations working in rural, remote, or under-served regions, together we address a myriad of health challenges.

Interim Reporting
The interim financial and evaluation materials will be emailed to the organization well in advance of the reporting due date. For one-year grants, interim reports are due 30 days after the mid-point of the grant. For multi-year grants, interim reports are due 30 days after the completion of each year of the grant term. For multi-year grants, the subsequent grant payment will be given after the interim reports have been reviewed by the Foundation.
Final Reporting
As with interim reports, the Foundation will email the final financial and evaluation report materials in advance of the reporting due date. Final report materials are due 60 days after the grant end-date. The last grant payment will be paid once the final reports have been reviewed by the Foundation.
Program, Financial, and Time Extension Requests
Please inform us as early as possible about any potential program, budget, or timeline changes. Most modifications can be accommodated if the Foundation is given justified and advanced notice.
